Free COVID Vaccine for Youth in Foster Care and Caregivers
In partnership with Board Supervisor Solis, we are hosting a vaccination clinic at CLC’s Monterey Park office on Saturday, May 15th for youth in foster care and caregivers. Second dose will be available at the CLC offices three weeks after.

CLC’s Monterey Park Office (101 Centre Plaza Drive Monterey Park, CA 91754)
REQUIREMENTS: You must be 16+ years old and provide photo ID. If you do not have a photo ID and are a CLC client, we will assist you with identification. For youth under 18, you will need permission from your guardian. Additional information provided below. You will not be asked about your immigration status when you get a COVID-19 vaccine. For youth in need of transportation, CLC can provide assistance.

Additional Information About Your Vaccine Appointment
Please review the Pre-Screening and Consent Form. Each patient will need to sign at the time of their vaccine. Our staff will assist with this consent process.
Regarding Minors:
For patients that are 12-17 years old, a parent or caregiver MUST sign a consent in order for the minor to receive the vaccine. Using the minor consent form, the guardian must attest that the patient meets the appropriate age requirements for vaccination and that they consent to the minor receiving the vaccine. The parent or guardian must also be present with the minor for the vaccination appointment. This is to provide support and decision-making should there be a need for medical treatment.
Patients aged 12-17 years old are only eligible to receive the Pfizer-BioNTech vaccine at this time. Moderna and J&J vaccines are only available to patients 18 and older.
